CDR Personal Care subscale (0–3). Rates the patient's ability to manage personal hygiene, dressing, and grooming independently. 0 = fully independent, 3 = requires complete assistance. Unlike the other CDR domains, Personal Care has no half-point option.

MMSE — a 30-point cognitive screening test covering orientation, memory, attention, language, and visuospatial ability. Score ≥24 is generally normal; lower scores suggest cognitive impairment.

Alzheimer's Disease Assessment Scale — 13 tasks (0–85). Extends ADAS11 with concentration and delayed word recall. Lower score = better cognitive performance.

Alzheimer's Disease Assessment Scale — 11 tasks (0–70). Measures memory, language, and praxis. Lower score = better performance.

MoCA — a 30-point screening tool for mild cognitive impairment covering visuospatial, naming, attention, language, abstraction, delayed recall, and orientation. Higher = better.
